Step 01

Talk with our bariatric nurse

Call 772.794.1437 or attend one of our monthly weight loss seminars, which are held on the first Tuesday of every month at Cleveland Clinic Indian River Hospital.

Step 02

Complete an application

Download a patient application, then scan and email your completed form to Your application will be carefully evaluated to determine if you are a candidate for surgery and our bariatric nurse will call or email you once your application has been reviewed to schedule an appointment.

Step 03

Meet with our team

You’ll sit down with our bariatric nurse and registered dietician, as well as our bariatric surgeon. You and your surgeon will discuss the surgical or non-surgical solution that is best for you. Our goal is to ensure that you are successful and become the healthiest version of you!

Step 04

Obtain pre-operative clearance

It is a requirement that you have a primary care physician. If you do not currently have a primary care physician, Cleveland Clinic Indian River Hospital has a large network of physicians ready to meet your primary care needs. Based on individual risk factors and health history, certain patients may require preoperative clearance from a cardiologist and/or pulmonologist; we can assist you obtaining those clearances prior to surgery.

Step 05

Attend two pre-operative classes

You’ll attend a pre-op nutrition class led by our registered dietitian, as well as a pre-op education class led by our bariatric nurse. You’ll learn about the final preparations for surgery, including what to bring to the hospital, the importance of a preoperative two-week diet and what to expect after the surgery.
